FT-322 Log action at Awaiting Applicant state

JIRA Reference: FT-322

If the applicant makes a yes/no selection of whether they want to review the dossier at State 111, do the following:

• if yes
  • log action where ID = 62
• if no
  • log action where ID = 65 (create db record in ACTION table)
    • ID = 65
    • action_enabled = 'Y'
    • action_code = 'review.dossier.no'
    • action_description = 'Applicant chose not to review dossier'
• delete the batch job created for JIRA FT-300
