    • I was not able to consult
    • I did not request consultation
  • the second option will unhide an input box to give a reason why
    the applicant was not able to consult

  • Both checkboxes must be filled in order to submit the application

Below is a list of questions to be addressed as a result of this requirements document:

6-1-2017: Bev from Faculty Human Resources at Manoa said that they may need option C but do not require A and B.  Will revisit when Manoa is consulted in Phase II
